Incident report
The root cause of this incident was a storage subsystem failure, which again caused an index failure driving an extreme high read activity (increased 10000 fold) causing the system to become unresponsive as message processing almost went to a halt.
Actions
- Set up additional alerting around disk activity.
- Investigate storage subsystem failure further.
